SB 54 supplier data template (Excel and CSV)
This free template gives you one place to record packaging data for every component and to track supplier requests. It includes an Excel workbook and a CSV of the main sheet.
It is built for California SB 54 supplier data and works as a starting point for other packaging EPR states. It is general information, and you should confirm what each state requires.
What is inside the template?
- Instructions. How to use the workbook and what each column means.
- Component Data. One row per component: SKU, product name, component, supplier, material, coating or layers, weight per unit (g), units sold into state, plastic component (Y/N), recycled content %, source document, date received and notes.
- Supplier Tracker. Supplier, contact, requested on, follow-up dates and status.
- Example rows. Three example rows are labeled “Example” so you can see how a finished row looks. Delete them before you start.

How do you use it?
- Add one row per packaging component for each SKU you sell into the state.
- Ask suppliers for material, coating or layers, weight and recycled content. Our guide to requesting packaging specs from suppliers has a sample email.
- Record the source document and the date next to every value.
- Use the Supplier Tracker to log requests and follow-ups.
For what each field means, see SB 54 supply report data, or start with the California SB 54 guide.
